These Terms of Service ("Terms") govern your use of adjustedatx.com and any online booking, intake, or messaging tools linked from it (together, the "Site"). The Site is operated by Hardwick Chiropractic LLC, doing business as Adjusted ATX ("Adjusted ATX," "we," "us," or "our"), 9015 Mountain Ridge Drive, Suite 120, Austin, TX 78759.

By using the Site, booking an appointment, or submitting a form, you agree to these Terms and to our Privacy Policy. If you do not agree, please do not use the Site.

1. Site content is not medical advice

The Site's articles, symptom pages, videos, and blog posts are general education only. They are not a diagnosis, treatment plan, or substitute for an in-person examination by a licensed chiropractor or other healthcare provider.

Using the Site, sending a message, or booking online does not create a doctor–patient relationship. That relationship begins only after an in-office consultation and examination, and our acceptance of you as a patient.

Do not delay or ignore professional medical advice because of something you read on the Site.

The Site is not for emergencies. If you have a medical emergency (for example, sudden weakness, numbness, loss of bladder or bowel control, chest pain, or severe trauma), call 911 or go to the nearest emergency room.

2. Appointments and online booking

You can request appointments through our online scheduling tool or by calling (512) 373-8181. An online booking is a request until it is confirmed; we may reschedule or decline a booking, for example if the time is no longer available or care at our office is not appropriate for you.

Accurate details. You agree to give accurate contact and booking information.

Cancellations and rescheduling. Please give at least 24 hours' notice to cancel or reschedule.

Late arrivals and no-shows. If you arrive more than 15 minutes late, we may need to shorten or reschedule your visit. Missed appointments and cancellations with less than 24 hours' notice may be subject to a $50 fee, and repeated no-shows may result in loss of online booking privileges or promotional pricing.

Third-party scheduling. Our booking tool may be provided by a third party. Your use of it may also be subject to that provider's terms and privacy policy.

3. Offers, fees, payment and insurance

$47 New Patient Special. The special includes a consultation, chiropractic exam, adjustment (if clinically appropriate), report of findings, and care recommendations. Unless stated otherwise:

It is for new patients only, one per person, and has no cash value.

It cannot be combined with other offers.

It is not available to patients whose visit is billed to Medicare, Medicaid, or other government healthcare programs, or where prohibited by an insurance contract or law.

Imaging, additional therapies, and follow-up visits are not included.

We may change or end any offer at any time; the offer shown when you book applies to that booking.

Pricing. Prices on the Site may change without notice. All payment arrangements and care plans are agreed in the office before care begins.

Insurance. As a courtesy, we may verify your benefits and submit claims for you. A benefits check is not a guarantee of payment. You are responsible for deductibles, co-insurance, co-pays, and any services your plan does not pay for. HSA, HRA, and FSA funds may be used where your plan allows.

Auto accident, work injury, and Medicare care. Coverage for these cases depends on your claim, your carrier, and program rules. Statements on the Site about coverage are general descriptions, not a promise that any particular claim will be paid in full. You agree to provide accurate claim, insurer, and attorney information, and you remain responsible for any balance not paid by a third party unless a written agreement says otherwise.

Refunds. If you prepay for a care plan and stop care early, you may request a refund for unused visits. Visits already received are charged at our regular per-visit rate, and a $25 administrative fee is deducted from the refund.

4. Intake forms, health information and minors

You agree that health history and other information you submit through our intake forms is accurate and complete to the best of your knowledge, and that you will tell us about changes. Incomplete or inaccurate information can affect the safety of your care.

Health information. Protected health information you give us as a patient is handled under HIPAA and our Notice of Privacy Practices, which you will receive at your first visit. Website usage data is covered by our Privacy Policy.

Do not send sensitive health details through general contact forms, email, social media, or text unless we have told you that channel is secure.

Minors. We offer pediatric care, but the Site is intended for adults. A parent or legal guardian must book appointments, complete forms, and give consent for any patient under 18.

5. Calls, texts and emails

By giving us your phone number or email, you agree that we may contact you about your appointments, intake forms, billing, and care, including by automated text message and email.

Marketing messages (such as offers or newsletters) are sent only with your consent, and consent is never a condition of receiving care.

Message frequency varies. Message and data rates may apply.

Reply STOP to opt out of texts or HELP for help, or use the unsubscribe link in any email. You may still receive essential messages about scheduled care unless you ask us to stop.

6. Testimonials, reviews and results

The Site displays reviews and testimonials from patients, including reviews posted on Google. They reflect individual experiences.

Results vary from person to person, and no specific

outcome is promised or guaranteed. Any description of conditions we help with is not a claim that chiropractic care will cure or resolve that condition for you.

If you post a review or comment about us, you are responsible for it and must not include false, defamatory, or another person's private information.

7. Use of the Site

Our content. The Site's text, logos, photos, videos, and design belong to Adjusted ATX or its licensors. You may view and print pages for personal, non-commercial use. You may not copy, republish, or sell Site content without our written permission.

Acceptable use. You agree not to:

book appointments or submit forms in someone else's name without authority;

interfere with the Site's security or operation, or upload malicious code;

scrape, harvest, or collect data from the Site by automated means;

use the Site for any unlawful purpose.

Third-party links and services. The Site links to services we do not control, such as Google Maps, Google reviews, Facebook, Instagram, YouTube, and our scheduling provider. We are not responsible for their content or practices; your use of them is governed by their own terms.

8. Disclaimers and limitation of liability

The Site is provided "as is" and "as available." To the fullest extent permitted by law, we disclaim all warranties about the Site, including that it will be accurate, complete, uninterrupted, or error-free.

To the fullest extent permitted by law, Adjusted ATX and its doctors, staff, and contractors will not be liable for any indirect, incidental, or consequential damages arising from your use of, or inability to use, the Site. Our total liability for any claim relating to the Site will not exceed $100.

This section applies to the Site only. It does not limit any rights or remedies you have under Texas law relating to the professional chiropractic care you receive in our office.

Indemnity. You agree to indemnify Adjusted ATX against claims arising from your misuse of the Site or your violation of these Terms.

9. Governing law, changes and contact

Governing law. These Terms are governed by the laws of the State of Texas. Any dispute relating to the Site will be brought in the state or federal courts located in Travis County, Texas.

Changes. We may update these Terms from time to time. The "Last updated" date shows when they last changed. Continued use of the Site after changes are posted means you accept the updated Terms.

Severability. If any part of these Terms is found unenforceable, the rest remains in effect.
